Overview

We are looking for a Social Worker to join our company, where you’ll deliver critical social work services to support functionally impaired adults through structured and compassionate care in a secure, community-based setting.

Key Responsibilities:

• Conduct initial and ongoing assessments of participants to inform the individual plan of care, as part of a multidisciplinary team. Legal Information Institute
• Maintain signed progress records in each participant’s file on a quarterly basis. Legal Information Institute
• Implement and oversee discharge planning, ensuring coordinated and appropriate transitions. Legal Information Institute
• Facilitate referrals to external resources and deliver counseling when necessary. Legal Information Institute
• Lead or arrange problem-oriented discussion groups tailored to participants’ needs. Legal Information Institute
• Act as a liaison between participants, their families, and home environments. Legal Information Institute
• Conduct referral and follow-up for individuals who may not be appropriate for adult day health care. Legal Information Institute
• Ensure privacy accommodations for counseling by providing an appropriate space. Legal Information Institute

Required Qualifications

• Licensed Social Worker: Certified and Licensed Social Worker
• Demonstrated experience in community-based social services, ideally working with aging or functionally impaired adults.
• Competency in assessment, care planning, progress documentation, and coordination of services.
• Strong interpersonal, counseling, and facilitation skills for group sessions.
• Ability to collaborate effectively within a multidisciplinary environment.
• High degree of organization and discretion, especially handling confidential and sensitive information.

Preferred Qualifications

• Prior experience in adult day health, long-term care, or geriatric social work.
• Familiarity with California regulatory frameworks, particularly Title 22 licensing and practice standards.

Job Type: Full-time

Pay: $33.00 - $40.00 per hour

Expected hours: 35 per week
