Education & Patient Care:

• Educates patient/caregiver to facilitate discharge
• Provides and reinforces individualized patient/caregiver education
• Facilitates discharge and contingency planning
• Patient Age Groups: Adolescents, Adults, Geriatrics
• Patient Ratio: 1:35 on days, 1:50 on weekends

• Patient Diagnoses: Special procedures/unit details: Case Management assistant can be an LVN or unlicensed with experience

Job Overview: Seeking a dedicated Discharge Planner Assistant with at least one year of experience to work a day shift with every third weekend coverage. The successful candidate will be responsible for a variety of tasks related to patient discharge, including education, planning, and coordination of care. Experience with the Epic charting system is required.

Pay: $24.00 - $28.00 per hour
