Novum Behavioral Health Systems is seeking an Embedded Case Manager to join our team. This position is responsible to ensure and/or deliver and properly document the provisions of care management services including facilitating the patient’s engagement in services with core providers, social services agencies, external care management organization, coordination with provider/facility and hospital case managers and discharge planners. Ensure coordination with entitlement programs and facilitating social support networks such as clean and sober living homes, therapeutic living environments, food banks and other entitlement programs.

Responsibilities/Duties: (but not limited to)

· Collaborating and coordinating services

· Coordinate the care management needs of the member with the patient’s provider, facility, hospital, PCP, Specialists and others involved in care

· Experience working in a hospital setting and working in coordination with doctors, nurses, social workers and discharge planners

· Flexibility to attend multiple inpatient settings same day to collect information and proactively monitor member progress towards treatment goals/discharge

· Participate in Care Management/Treatment Team Meetings

· Uphold the organization’s ethics, code of conduct and customer services standards.

Knowledge, Skills and Abilities:

· Knowledge of treatment plans.

· Good organizational and time management skills

· Ability to write clear, concise documentation for record-keeping and billing purposes

· Strong problem solving and communications skills (verbal and written)

· Excellent crisis management skills

· Ability to use electronic communications and internet resources.

Education, Licenses, Certifications and Experience:

· Minimum of Bachelor’s Degree in a human services field
