Job Title: LMSW (Licensed Master Social Worker) / RN Case Manager – Discharge Planner
Location:110 Irving St NW, Washington, DC 20010
Contract Duration: 13 Weeks
Shift: Day, 5x8-Hour (08:00 AM – 04:30 PM)

Requirements

• LMSW/LGSW/LCSW – pending DC licensure accepted with proof of application.
• RN DC Licensure – pending DC licensure accepted with proof of application.
• Certifications: BLS required.
• Minimum 2 years acute hospital discharge planning experience.
• Experience with admission criteria review, benefits eligibility, care coordination, and DME ordering..
• Preferred Experience: Teaching hospital, community hospital, LTAC, Trauma Level I/II.
• Travel Experience Required: Yes

Pay: $2,200.00 - $2,285.00 per week
