Position Summary
The Licensed Practical Nurse (LPN) – Care Coordinator is responsible for ensuring high-quality, compliant, and personalized care for Senior Nannies’ clients. This role involves managing client relationships, developing and updating care plans, conducting on-site visits, and ensuring caregiver performance meets company and regulatory standards. The Care Coordinator serves as a key link between clients, caregivers, and the Senior Nannies management team.
Key ResponsibilitiesCare Coordination
- Serve as the primary clinical point of contact for assigned clients.
- Conduct initial intake assessments and collaborate with RNs to develop individualized care plans.
- Perform follow-up visits and reassessments to ensure care plans meet client needs.
- Communicate promptly with clients, families, and providers about changes in care or service needs.
- Provide ongoing clinical guidance, education, and feedback to caregivers.
Compliance & Documentation
- Ensure all documentation, care plans, and visit records meet AHCA, Medicaid, LTC insurance, and Senior Nannies’ standards.
- Maintain accurate client records in the EHR system (Kantime/WellSky).
- Participate in internal audits and quality improvement initiatives.
Collaboration & Communication
- Serve as the bridge between clinical oversight and day-to-day operations.
- Provide input on policy updates and process improvements.
Qualifications
Required:
- Active Florida LPN license in good standing.
- 2+ years of experience in home healthcare, geriatric care, or care coordination.
- Proficiency in EHR software (Kantime or WellSky preferred).
- Strong understanding of AHCA regulations and home care compliance standards.
- Excellent verbal, written, and organizational skills.
