Access Intake Counselor

Crossroads Health provides a continuum of quality life-changing behavioral health and specialized chemical dependency treatment services to children, adolescents, young adults, adults and families. As a leader in the behavioral health field, we offer a vast array of programs and services that evaluate, educate, strengthen and support thousands of children, adults and their families each year.

Our Intake and Access Team is currently looking for an Access (Intake) Counselor to join the team. The Access Counselor provides Diagnostic Assessments and also carries a caseload of counseling clients. This position is responsible for assessment, diagnosis, and referral as well as counseling across youth, adolescents, and adults. This licensed mental health provider will engage as a member of a collaborative team.

More specifically, this position is responsible for:

  • Providing trauma informed assessments and counseling to clients and their family system in the office.
  • Providing accurate, concise, and concurrent documentation according to program procedures and policies. Including, but not limited to Diagnostic Assessment, diagnosis, treatment plans, progress notes and other intake documents.
  • Interviewing all new clients to conduct thorough diagnostic assessments.
  • Providing and coordinating crisis intervention and triage services.
  • Managing client records in accordance with Ohio MHAS, Joint Commission standards and Crossroads Health's case record procedures in a timely, professional, accurate, and legible manner
  • Developing and maintaining positive relationships with referral sources and community providers

Position Requirements:

  • Bachelor's Degree in Social Work
  • Master's degree in Social Work/Mental Health or related field (preferred)
  • LPC/LSW/MFT required. LISW preferred.
  • Experience performing Diagnostic Assessments in a mental health setting (preferred)
  • Experience in chemical dependency assessment (preferred)
  • Experience in crisis intervention, triage, and knowledge of community resources (preferred)
  • Provide consultation and collaboration with community partners and clients' treatment teams.
