Minimum Qualifications:
  • Possesses, at a minimum, an Associate’s Degree. A bachelor’s degree from an approved school of Social Work and licensed in the State of Ohio would be of benefit.
  • Two years of experience in a health care setting working directly with individuals preferred.
  • Is able to read, write, and communicate effectively the English language; has sight/hearing senses or uses prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
  • Is able and willing to work independently with flexibility and integrity, as well as effectively and harmoniously with team members, residents, family members, support agencies, government agencies/personnel, and the general public.
  • Possesses leadership ability and is able to make independent decisions when circumstances warrant such action.
  • Has patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle difficult residents.
  • Is able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the Community.
  • Must be fully up-to-date with required vaccinations.
As a Social Service Coordinator you can expect to:
  • Is responsible for completing and maintaining documentation which meets ERS, federal, state and local regulations, (i.e. resident comprehensive assessment, progress notes, plan of care, resident rights, advanced directives, MDS/RAP’s, CAA’s PASRR, and etc.).
  • Refers residents/families to appropriate social service agencies when ERS does not provide the services.
  • Assists residents/families to resolve social and environmental challenges including but not limited to financial needs, including providing information on Medicare/Medicaid, discharge planning, challenges related to clothing and possessions, transportation needs, relationship difficulties. Provides information and refers to internal or community resources as needed.
  • Advocates for residents and families with regard to concerns, needs or problems, including assisting residents/families with anxieties and stress caused by illness and admission to the Community, difficulties in coping with residual physical and/or cognitive disabilities, fears related to helplessness and death and the need for institutional and/or specialized care.
  • Participates in planning, development, and implementation of care plans and resident assessments as necessary.
  • Schedules and facilitates care conferences in accordance with required regulations and as needed; participates in resident reviews and level of living meetings..
  • Assists in the coordination of admissions, room moves, transfers, and discharges; completes transfer and discharge notifications for assigned residents in accordance with regulatory guidelines.
  • Schedules and leads monthly resident council meetings for assigned nursing households and documents meeting notes following each meeting. Shares feedback with managers on topics discussed during meetings.
  • Develops/facilitates structured groups/individual counseling as needed.
  • Assists in reviews of complaints and grievances made by the resident/family and makes a written/oral report to the Administrator or designee, indicating what actions were taken to resolve the complaint or grievance. Facilitates grievance committee.
  • Works with residents to sign up for specialist visits such as podiatry, audiology, optometry, psychology, and psychiatry for the community and coordinates these visits with the providers.
  • Assists in the development, administration and coordination of department policies and procedures.
  • Participates in community planning related to the interests of the Community and the services and needs of the residents and families.
  • Attends and participates in continuing educational programs designed to keep current with changes in social services, as well as to maintain your license in an active status.
  • Participates in resident/family council meetings as requested and provides support services to such councils.
  • Keeps abreast of current federal, state, and local regulations, as well as professional standards and make recommendations on changes in policies and procedures.
  • Serves and/or leads various committees of the Community as directed.
  • Speaks at civic functions and organizational meetings as required or invited.
  • Works with the Community consultants as necessary and implements recommended changes as required.
