Job Title: Transition Specialist/Discharge Planner
Reports To: Director/ Assistant Director

JOB SUMMARY: The Transition Specialist works as a member of an interdisciplinary treatment team to develop and increase client’s knowledge of community services, how to access such services and the importance of these services. This position involves participation and implementation of program goals consistent with preventative and holistic care, having objectives to promote a strength- based, wellness-informed client- centered approach to assisting individuals in achieving person-driven and self-management goals.
QUALIFICATIONS: To perform this job successfully, an individual must be at least 18 years of age and able to perform each essential duty satisfactorily. An individual must be able to perform their duties in a positive work mode and serve as a positive role model and influence for co-workers and peers. Employee shall be in good health, and shall be physically, mentally, and occupationally capable of performing assigned tasks. Minimum Qualifications
EDUCATION and/or EXPERIENCE: One year of full-time experience working in a related service entity serving persons with mental disabilities and/or substance use. If employee does not have a minimum of one (1) year of full-time experience working in a program serving people with a psychiatric diagnosis then a Plan of Supervision is required and will be put into place.
CERTIFICATES, LICENSES, REGISTRATIONS
Required: Valid State Driver’s License with appropriate endorsements.
Required: Valid First Aid/CPR certification (Can be completed after hire).
Required: AOD certification/registration
