JOB STANDARDS
  • Bachelor’s degree in a Human-Services related field;
  • OR a combination of education and experience sufficient to perform the duties of the position.
  • Must have good verbal and written communication skills.
  • Demonstrates regular and predictable attendance.
  • Consistently and continually represents the agency in a positive and professional manner and provides appropriate internal and external customer service.

ESSENTIAL FUNCTIONS
  • Conducts home visits with clients to perform assessments including personal and environmental safety, cognitive functioning, and coping skills.
  • Investigates Adult Protective Services (APS) cases including conducting face-to-face interviews with all parties of the case and collaterals.
  • Formulates care plans from assessments and observations maintaining the elder in the least restrictive setting.
  • Completes referrals to law enforcement as necessary.
  • Completes all investigation/assessment activities following Federal, State and County policies and procedures, within mandated timeframes.
  • Documents all work related activities in the ODAPS and Wellsky systems within the established timeframes.
  • Coordinates services with community partners.
  • Contacts clients periodically to assess needs and to ensure the level of service is appropriate, and quality is adequate.
  • Performs annual re-assessments.
  • Advocates for clients and educates clients regarding available services.
  • Completes additional reports as required such as Case Plans and Safety Assessments; completes County senior services enrollment forms (e.g. receipt of privacy practices, release of information, financial eligibility, etc.).
  • Attends and testifies in court hearings.
  • Participates in interdisciplinary team meetings and advocates for least restrictive and safe options for elders.
  • Completes community outreach activities to educate the public on Senior Sales Tax Levy services and participates on community boards that impact elder services.
  • Maintains required licensures and certification, if any.
