Description:
Part time Night shift (7p-7a) Emergency Department Social Worker. 2 12-hour shifts/week. Weekends (every 3rd) and holidays (1/year) required.

What You'll Bring:
Education: Master’s Degree in Social Work or a related field from an accredited institution.
Licensure: Must be eligible for licensure as a Limited Licensed Master Social Worker (LLMSW) or Licensed Master Social Worker (LMSW) by the Michigan Board of Social Work.
Experience: Minimum of 2 years of social work experience in a healthcare setting.

- Work Location: Trinity Health St. Mary’s Hospital, Grand Rapids, MI
- What you will work: Two 12 hour night shifts [7pm-7am], every third weekend and 1 holiday/year Position Summary: Join our compassionate team at Trinity Health St. Mary’s Hospital, where your skills in clinical social work will make a meaningful difference in the lives of our patients and their families. In this role, you will conduct psychosocial assessments, provide brief solution-focused counseling, and develop collaborative treatment goals. Your efforts will help patients overcome barriers, access community resources, and ensure continuity of care across inpatient and outpatient settings.
- Conduct comprehensive psychosocial assessments for patients and families.
- Develop treatment plans that address emotional, social, and resource-related needs.
- Provide brief, solution-focused counseling and facilitate discharge or care planning.
- Advocate for patients and coordinate referrals to community-based services.
- Collaborate effectively with interdisciplinary care teams.
- Document all assessments, plans, interventions, and outcomes clearly and promptly.
- Meet departmental productivity expectations and performance standards.
