Description
The Behavioral Health Services Coordinator plays a key role in connecting patients with essential behavioral health resources. Working closely with the Operations Team and Behavioral Health Integration Director, this position manages clinician schedules, processes referrals, and provides exceptional customer service to ensure patients receive timely, coordinated care. The coordinator also supports special projects, contributing to the overall efficiency and success of the Behavioral Health Department.
Duties
- Supports patients with scheduling Behavioral Health Department appointments and care coordination.
- Identifies patients who are likely to benefit from Behavioral Health services in a systematic way, prior to patient visit.
- Assists with Behavioral Health special projects, coordinating with Outreach Programs, and maintaining care continuum communication efforts.
- Follow-up contact via phone with patients to reschedule appointments or assist with engagement in services.
- Answers all email and voicemail messages in a timely manner.
- Interacts politely and as a team player with all NCHC employees, providers, patients, and public.
- Promotes positive patient relation in accordance with NCHC policies, providing a high level of quality in personal attention and service to patients and visitors.
- Maintains confidentiality of patient information.
- Performs other duties as assigned.
Requirements
- 1 to 2 Years Customer Service Experience Required and
- Behavioral Health Technician Experience/Training Preferred
- Behavioral Health Technician Certificate Upon Hire Preferred
- Medical Interpreter Certification or Ability to Obtain Upon Hire Preferred
- Basic Life Support Upon Hire Required
