We are seeking a dedicated professional to join our team as a Discharge Planner or Licensed Social Worker. This role involves working closely with patients to ensure a smooth transition from hospital to home or other care facilities.

Shift Details

• Shift Hours: 8:00 AM - 4:30 PM (5 days a week)
• Weekend Rotation: Every 5th or 6th weekend
• Holiday Coverage: As needed
• No on-call or floating requirements
  
  

Requirements

• Eligibility for LMSW/LGSW/LCSW with exam passed
• Accepts RN Case Manager/Discharge Planner or Licensed Social Worker
• DC RN Licensure or pending with proof of application
• BLS Certification
• Minimum 2 years of discharge planning experience, including:
• Admission criteria review
• Benefits eligibility
• Care coordination
• Conducting needs assessments
• Ordering DME
• Experience with home health, hospice placement, LTAC, nursing home placement, rehab placement
• Acute hospital experience
• Prior charge experience preferred
• Completion of at least 1 travel assignment
• Regulatory Skills: CMS, HIPPA, JCC; workers compensation preferred
