Overview: Ascendence Health Services is seeking a dedicated and compassionate Licensed Professional Clinical Counselor (LPCC) or Licensed Independent Social Worker (LISW) to join our mental health team. The ideal candidate will be responsible for running mental health group counseling sessions, diagnosing, treating, and creating treatment plans for individual clients within a long-term care nursing facility. The successful candidate will have a strong commitment to promoting mental wellness and providing high-quality therapeutic support.
Responsibilities:
- Group Counseling: Facilitate engaging and supportive group therapy sessions, fostering a safe environment for participants to share experiences and learn coping strategies.
- Individual Counseling: Conduct thorough assessments to diagnose clients’ mental health conditions and develop personalized treatment plans that address their unique needs and goals.
- Treatment Planning: Collaborate with clients to establish clear, measurable treatment goals and regularly review progress, adjusting plans as necessary to ensure effective care.
- Documentation: Maintain accurate and timely documentation of client interactions, treatment plans, and progress notes in compliance with legal and ethical standards.
- Crisis Intervention: Provide immediate support and intervention during crisis situations, including risk assessments and safety planning.
- Community Outreach: Participate in community education and outreach programs to raise awareness about mental health issues and available resources.
- Collaboration: Work closely with other mental health professionals, including psychiatrists, social workers, and support staff, to coordinate integrated care for clients.
- Professional Development: Stay current on best practices in mental health treatment and participate in ongoing training and professional development opportunities.
Qualifications:
- Master’s degree in Counseling, Social Work, or a related field.
- Current and valid LPCC or LISW licensure in Ohio.
- Currently credentialed as a Medicaid and Medicare provider.
- Experience in Mental health counseling or social work.
- Strong understanding of group dynamics and therapeutic techniques.
- Excellent interpersonal and communication skills, with the ability to build rapport with diverse populations.
- Knowledge of mental health diagnoses, treatment modalities, and ethical standards in clinical practice.
- Ability to work independently as well as part of a collaborative team.
- Proficient in electronic health record (EHR) systems and Microsoft Office Suite.
Preferred Qualifications:
- Experience running group therapy sessions.
- Additional certifications in specialized areas of mental health (e.g., trauma-informed care, DBT, CBT).
