Chronic Care Management Patient Coordinator

You will be documenting patient information for their health care plans in the clinics EMR system. Following up with patients on their appointments, medications, and specialty visits. Reviewing tasks for the patient. Consulting with the doctor on patients needs and questions that need immediate assistance. This position is in the clinic.

Medical Office Experience

Patient Care Experience.

EHR experience necessary

Proficiency in Office, Word, Excel and Outlook

Monday through Friday 8-5

Must be a self starter and self sufficient with a drive to exceed.

Reliable transportation needed

Pay:$15 per hour
