About Job
The Social Worker’s primary purpose will be to work in conjunction with all of the health services within Mostellar Medical Center in order to develop a comprehensive service delivery system aimed at meeting the needs of the patients. This position demands the ability to interact with Federal, State, and local agencies in order to maximize resources and to coordinate program development goals.
- Supports all facets of social services for children and adult patients of the Mostellar Medical Center.
- Supports all facets of the services for children with local agencies and other programs for children and adults to provide additional service elements.
- Develops and initiates programs that provide services to children of identified parents/patients. (example: early childhood intervention, developmental day care, child service coordination.)
- Collaborate with the medical staff to provide effective, quality services to the patients of the health center target population.
- Assures program records conform to area Mental Health standards/Health Department standards and meet Medicaid requirements.
- Performs miscellaneous job-related duties as assigned.
- Participates as a member of a patient care team that involves clinical and non-clinical personnel.
- Interacts with patients, families and other members of the healthcare team working to achieve stated objectives and high-quality patient outcomes.
- Communicates with specialists, healthcare facilities, and community organizations for patient care coordination.
- Effectively communicates with all patient populations and identifies patient needs; particularly vulnerable populations.
- Uses evidence-based approach to patient self-management support of health.
- Identifies patient population health needs based on diagnosis or preventative measure requirements.
- Participates in quality improvement and action plan development.
- Assists and coordinates crisis management as needed.
