Job Title: Licensed Clinical Social Worker (LCSW) – Acute Care Hospital

Job Summary:

We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our multidisciplinary team in an acute care hospital setting. The LCSW will play a vital role in providing psychosocial assessments, emotional support, crisis intervention, discharge planning, and advocacy for patients and their families. This position ensures patients receive holistic, patient-centered care and a smooth transition from hospital to post-acute care services.

Key Responsibilities:

Assessment & Care Planning

• Conduct comprehensive psychosocial assessments to evaluate patients’ emotional, social, and financial needs, as well as those of their families.
• Develop individualized care plans in collaboration with patients and the medical team to address identified needs and support recovery goals.

Counseling & Crisis Intervention

• Provide emotional support and counseling to patients and families facing the stress of illness, hospitalization, or life-altering diagnoses.

MUST HAVE HOPITAL EXPERIENCE

Job Type: Contract

Pay: $45.70 - $51.77 per hour

Expected hours: 40 per week
