Location: Hemet Hills Post Acute Hemet, CA Schedule: Full-time | Flexible Hours Available

Pay: From $23 per hour +/ DOE LVN Preferred But Not Required

Hemet Hills Post Acute is a warm and welcoming skilled nursing facility in Hemet, CA, offering top-tier short and long-term care, rehabilitation, and respite services. As our Discharge Planner, you'll be the bridge between care and independence, helping residents transition smoothly from our facility to their next destinationwhether it's home, assisted living, or another care setting. Your role will include:

• Coordinating discharge plans with residents, families, and interdisciplinary teams
• Ensuring all medical, social, and logistical needs are met for a safe transition
• Communicating with external providers and community resources
• Educating residents and families on post-discharge care and services
• Keeping documentation accurate, timely, and compliant

What We're Looking For:

• Experience in discharge planning, case management, or social services
• LVN license preferred (but not required)
• Strong communication and organizational skills
• Compassionate, patient-centered mindset
• Ability to multitask and thrive in a fast-paced environment
