- Conduct biopsychosocial assessments, screenings, and develop individualized, culturally sensitive treatment plans for patients with substance use and mental health disorders.
- Deliver psychoeducation, counseling, and harm reduction strategies through individual and group sessions.
- Collaborate with care teams, including primary care providers and community organizations, and provide case management as needed.
- Maintain accurate documentation, ensure compliance with regulations, and participate in quality improvement activities.
- Utilize diagnostic tools like DSM V, and stay current with evidence-based practices.
- Requirements include a Master’s in Social Work or Mental Health Counseling, NYS licensure (LMSW or LMHC), and at least one year of relevant experience.
- Preferred experience in community healthcare; salary ranges from $59,000 to $68,000 annually.
