The Social Worker, LSW provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute or terminal illnesses. This position advises, educates, and counsels patients and their loved ones based on their needs and concerns. In addition, the Social Worker, LSW provides crisis intervention, refers patients to appropriate services, and assists patients and their loved ones in understanding the implications and complexities of the situation to ensure the patient and their loved ones receive continuity of care and services through resource management.

WHAT YOU CAN EXPECT
Schedule: Monday - Friday, 8am-5pm
Collaborate with multidisciplinary teams to evaluate and assess patient's medical and physical needs.
Refer patients to appropriate services if patients are identified with depression, anxiety, grief or mental illness.
Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy.
Identify environmental impediments to patient progress through interviews and review of patient records. Treatment Plans Assists patients and their caregivers in identifying goals of care and assists with unmet needs through linkages to community resources.
Assist in crisis intervention.
Document inpatient electronic medical records in a clear, concise, accurate, and timely manner.
QUALIFICATIONS
Bachelor's Degree in Social Services field required
Illinois LSW required
1 year of Social Services experience preferred
