Job Summary:

We are seeking a compassionate and motivated BSW Social Worker to join our team. The ideal candidate will provide support services to individuals and families, assisting with access to community resources, case coordination, and basic counseling. This role is ideal for recent graduates or entry-level professionals with a Bachelor's degree in Social Work and a passion for helping others to navigate social, emotional, and environmental challenges.

Key Responsibilities:

• Conduct psychosocial assessments and assist patients in identifying needs and goals.
• Connect individuals and families to appropriate community resources such as housing, food assistance, financial aid, transportation and healthcare services.
• Collaborate with healthcare providers, case managers, and community organizations to coordinate services.
• Advocate on behalf of patients to ensure access to services and support.
• Document all interactions, assessments, referrals, and follow-ups in accordance with company policies and confidentiality standards, and in the electronic medical record.
• Educate patients and families on coping strategies, support services, and rights.
• Participate in case reviews and interdisciplinary team meetings to ensure effective service delivery.

Qualifications:

• Bachelor’s degree in Social Work (BSW) from an accredited program.
• Strong interpersonal, communication, and problem-solving skills.
• Knowledge of community resources and social service systems.
• Ability to work independently and as part of a multidisciplinary team.
• Familiarity with documentation practices and basic case management systems.
• Valid driver’s license and reliable transportation may be required (depending on role).

Preferred Experience (Not Required):

• Internship or prior work experience in a healthcare, non-profit, or human services setting.
• Bilingual skills (e.g., Spanish, ASL) are a plus.
• Familiarity with Medicaid/Medicare, behavioral health, geriatric populations or child welfare systems.

Work Environment:

• Office-based, field work, and home visits depending on assignment.
• May involve occasional evening or weekend hours based on patient needs.

Pay: $69,000.00 - $76,000.00 per year
