Full Time LVN Clinical Care Coordinator

Must have CA VN License.

Must be able to Commute South Bay/Los Angeles areas.

Hourly $35-40/hr + Mileage Reimbursement.

Position Summary

The Clinical Care Coordinator is a nurse who engages in community outreach to counsel, explain and educate patients and families about hospice and palliative care services, reviews consents and other documents and assists with election of hospice benefit. The Clinical Care Coordinator coordinates patient discharge from hospital into hospice or palliative care programs and performs start of care and initial nursing assessment. The Clinical Care Coordinator works cooperatively in coordination with the hospice and/or palliative care teams. The Clinical Care Coordinator requires desire to work directly with patients and families to assist them efficiently accessing hospice and/or palliative care services. Must be able to utilize computers and be comfortable with electronic medical record. Must be able to meet with patients and families in person and/or remotely

Qualifications

  • Graduate of accredited school of nursing with current nursing license in California.
  • Certification in Hospice and Palliative Nursing encouraged.
  • At least one-year experience as a professional nurse within the last 3 years
  • Excellent verbal and written communication skills.
  • Spanish speaking a plus, but not required.
  • Valid driver’s license, and auto insurance, reliable and well-maintained transportation.
