What You'll Do
- Provide skilled home health Medical Social Work care: Perform in-home assessments: Evaluate geriatric patients' living situations, health, and resources.
- Connect to vital resources: Secure community aid like IHSS and Meals on Wheels for patients.
- Guide IHSS processes: Assist with applications and modifications.
- Document efficiently: Use our simple EMR system for all notes.
- Collaborate: Work with a supportive care team, ensuring coordinated care amongst supervising therapists, other disciplines, and within our administrative team to optimize patient outcomes.
What You Need
- A valid practicing license from the State of California LCSW/MSW
- CPR or BLS certification (within last 2 years)
- Driver’s license and auto insurance
- Physical + TB test (within the last year)
- Professional liability insurance
- Business license (from your local city, if applicable)
- Home Health Experience is a must
