We are currently seeking Care Coordinator Supervisor to join our School Based Mental Health program. Key Responsibilities
- The Care Coordinator Supervisor will guide, support and evaluate the Care Coordinator's individual efforts by focusing on ongoing clinical and compliance issues, staff development, concurrent clinical review and assistance to Care Coordinator with both daily caseload and crisis management.
- Maintains knowledge of Medicaid, of the behavioral health system and of community resources.
- Develops, monitors, and updates individual's person-centered care plans to ensure the individual's needs, preferences, health, and welfare.
- Assists individuals with connections to needed resources, including services, medical, social, educational, and other resources, regardless of funding source
- Identifies, reports, and addresses all incidents as defined in the program specific guidelines, following the process outlined by program specific guidelines.
- Documents all care coordination activities in a record to meet all requirements,
including accuracy, timeliness, and content requirements using the process prescribed by the program specific guidelines
Minimum Qualifications
- Possesses an active license as a Licensed Social Worker, Licensed Independent Social Worker or Registered Nurse; AND/OR Bachelor’s Degree in Nursing, Social Services or related field with a minimum of 2 years of full-time direct service experience. Experience must be relevant to the program.
- Will complete all training curriculums and professional development requirements as specified by the assigned program.
- Possesses integrity, self-discipline, with the ability to work independently within guidelines.
- Possesses knowledge of Medicare, Medicaid, insurance and waiver programs and community resources.
- Has great problem-solving skills.
- Possesses effective verbal and written communication skills with attention-to-detail in assessment and in documentation.
- Has familiarity with the suite of Microsoft Office programs.
- Possesses a valid driver’s license and car insurance as required by State law.
- Have experience working with youth, and their families, who meet a functional need for behavioral healthcare.
- Have knowledge of issues affecting individuals served and community-based interventions/resources for those individuals (e.g., Social Determinants of Health, etc.).
- Have effective interpersonal (oral and written) skills to interact professionally and ethically with all age groups from a diverse population, including differing intellectual, cultural, ethnic, psychosocial and socio-economic backgrounds.
- Have the ability to utilize critical thinking and independent judgment, along with multiple points of data/input to identify issues, develop solutions, and exercise judgment to meet requirements.
- Are familiar with suite of Microsoft Office programs.
- Possesses a valid driver’s license and car insurance as required by State law.
