The Licensed Clinical Social Worker (Behavioral Health Therapist, internally) works with Social Workers and Primary Care Providers, and our New Participant Advocate to identify those who could benefit from behavioral health support. Following the PACE care model, the Behavioral Health Therapist serves on a clinical interdisciplinary team to represent & advocate for each participant’s behavioral health needs. The Behavioral Health Therapist will support a panel of participants with their therapy needs. Core responsibilities include contributing to a center experience that participants want to spend time in, fostering a team culture that cares and creates joy, and an environment where all participants and team members belong. The role involves collaborating as part of the Interdisciplinary Team (IDT) to optimize health status and quality of life of the participants as part of the care plan, working in an integrated care environment with primary care providers and social workers, delivering and documenting behavioral health interventions including symptom management, counseling, crisis intervention, arranging resources and services, assisting with care transitions, providing individual and group counseling and care management. The therapist will facilitate individual and group therapies such as art and music therapy, psychosocial drama, counseling, and discussion to restore or expand participants’ social roles, provide training on behavioral health symptoms and interventions to Habitat Health staff, and perform related duties as assigned. Required qualifications include being licensed in clinical social work (LCSW), a minimum of two years experience working with the frail or elderly, experience coordinating and facilitating care conference meetings, proof of medical clearance for communicable diseases including TB test, up-to-date immunizations, current CPR/BLS certification or requirement to obtain within 30 days of employment, valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law. Preferred qualifications include bilingual skills (Spanish highly preferred, Russian/Vietnamese/Mandarin/Cantonese preferred) and previous experience assisting patients/participants with behavioral health and substance abuse issues. The position is onsite in Sacramento, CA.
