**Job Summary:**

The Per Diem Hospice Social Worker provides psychosocial support and counseling to hospice patients and their families, addressing their emotional, practical, and spiritual needs during the end-of-life transition. This position offers flexibility in scheduling, allowing MSW professionals to work on an as-needed basis.


**Responsibilities:**


1. **Assessment and Support:**

  • Conduct comprehensive psychosocial assessments of patients and their families to identify emotional, practical, and spiritual needs.
  • Provide counseling, emotional support, and crisis intervention to help patients and families cope with the challenges of terminal illness, grief, and loss.


2. **Care Planning and Advocacy:**

  • Collaborate with the interdisciplinary hospice team to develop individualized care plans that address the psychosocial needs of patients and families.
  • Advocate for patients and families by connecting them with community resources, financial assistance programs, and supportive services.


3. **Advance Care Planning:**

  • Facilitate discussions about advance directives, end-of-life preferences, and goals of care with patients and families.
  • Assist with completion of advance directives, including healthcare power of attorney and living will documents.


4. **Bereavement Support:**

  • Provide bereavement support to family members and caregivers during the anticipatory grief period and after the death of a loved one.
  • Coordinate grief support groups, memorial services, and other bereavement resources as needed.


5. **Documentation and Reporting:**

  • Maintain accurate and confidential documentation of all social work interventions, assessments, and interactions with patients and families.
  • Complete required documentation in electronic health record (EHR) systems and other hospice-specific databases.


**Qualifications:**


  • Master's degree in Social Work (MSW) from an accredited program.
  • Current licensure or certification as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) in the state of practice.
  • Minimum of [X] years of experience in social work, preferably in hospice, palliative care, or a related field.
  • Strong assessment, counseling, and crisis intervention skills.
  • Compassionate bedside manner and ability to provide emotional support to patients and families.
  • Excellent communication and interpersonal skills.
  • Proficiency in electronic health record (EHR) documentation.


**Working Conditions:**


  • Per diem scheduling allows for flexibility in hours and shifts, depending on the needs of the hospice organization and availability of the social worker.
  • Work settings may include patient homes, hospice facilities, nursing homes, or other healthcare settings.
  • Some evening, weekend, and holiday shifts may be required based on patient needs and staffing requirements.
