Essential Functions
- Completes initial as well as ongoing assessments of the psychosocial needs of all assigned patients/families
- Develops plan of care for patient/family that is tailored to individual needs and includes the interdisciplinary team as appropriate
- Reports patient/family contacts and interventions to other members of the interdisciplinary team appropriately
- Attends interdisciplinary team meetings, staff meetings, and other meetings as indicated
- Provides accurate documentation of each contact with or regarding the patient/family in accordance with current regulations
- Provides interventions as indicated in the interdisciplinary team care plan
- Completes a suicide assessment and bereavement risk assessment on all patients at the time of the initial psychosocial assessment
- Completes assessments for: physical abuse, rape, domestic abuse, sexual molestation, exploitation when appropriate
- Reports child abuse/neglect and dependent/elder adult abuse as required by law.
- Provides or arranges for individual or family counseling services to assist with the stress associated with a life-threatening illness
- Identifies and facilitates referrals to appropriate community resources and coordinates patient and family use of these resources
- Assists with benefit programs and end of life planning
- Develops and/or participates in education programs and workshops for patient/families, staff, and community at large
- Participates as a proactive and supportive member of the interdisciplinary team
- Provides input to program coordinator as to the ongoing clinical needs of the program as well as issues of quality improvement
- Attends and presents appropriate patient-related information at the Interdisciplinary Team Meetings that focuses on open care plan problems
- Adheres to federal and state regulations, as well as accreditation standards
- Completes and submits patient-related documentation according to Agency standards
- Responsible for following the Agency’s process for effective “hand off” communication
Experience and Education
- Master of Social Work degree from a school accredited by the Council on Social Work Education
- One-year social work experience in a health care setting preferred, experience in Long Term Care setting a plus
- Clinical experience with geriatric, AIDS, oncology, or terminally ill populations desirable
- Current CPR certification
- Bilingual (Spanish) skills preferred

Pay: $38.00 - $44.00 per hour
