About Job

At Firefly Home Care LLC, we are looking for a compassionate, detail-oriented, and highly organized Care Coordinator to join our team. In this vital role, you will play a key part in ensuring our clients receive the highest level of care, all while overseeing and supervising a dedicated team of caregivers. Your ability to coordinate care efficiently, support caregivers, and be a resource for our clients will help make a meaningful difference in the lives of those we serve.

Key Responsibilities:

  • Supervise Caregivers: Oversee caregivers, ensuring they follow company policies and procedures and deliver excellent care to clients.
  • Client Intake & Home Evaluations: Travel to clients' homes for initial intakes and ongoing home evaluations, ensuring a personalized care approach.
  • Review & Update Care Plans: Regularly update care plans and visit schedules to reflect clients’ evolving needs and preferences.
  • Coordinate Care Across Teams: Work closely with internal teams and external providers to ensure coordinated care for clients at every stage of their journey.
  • Manage Client & Caregiver Files: Maintain up-to-date client and caregiver files, ensuring compliance with policies and regulations.
  • Handle Inbound Calls: Professionally receive and triage inbound calls, addressing client and caregiver needs promptly.
  • Caregiver Onboarding & Training: Assist in caregiver onboarding, providing orientation and ongoing training to ensure the highest standards of care.
  • On-Call Rotation: Participate in an on-call rotation to support clients and caregivers during non-business hours.
  • Assist with Agency Operations: Support various agency operations to ensure smooth functioning.

Qualifications:

  • Organized & Detail-Oriented: Ability to manage multiple tasks with precision and attention to detail.
  • Fast-Paced Environment: Thrive in a busy, dynamic office environment and handle shifting priorities with ease.
  • Strong Computer Skills: Proficiency in email, Microsoft Office, web searches, and related tools.
  • Excellent Communication: Outstanding verbal and written communication skills for email, phone, and face-to-face interactions.
  • Customer-First Attitude: A genuine passion for delivering top-notch customer service and providing the best possible care to our clients.
  • Administrative Experience: Previous office administrative experience strongly preferred.
  • Solid Work History & References: A proven track record of reliability and responsibility.
  • Preferred Experience: Healthcare or home care administrative experience is strongly preferred.
