Care Manager Oncology | 7:00 AM - 7:30 PM (OCN Certification Required)

Full-Time | Sign-On Bonus: $20,000

About the Role:

Care Managers at Lexington Medical Center play a vital role in coordinating holistic patient care, providing leadership, mentorship, and advocacy while working within specialized medical units.

As a Care Manager, you will collaborate with multidisciplinary teams to ensure evidence-based, high-quality care in a fast-paced, patient-focused environment.

Minimum Qualifications:

Education: BSN required (ADN or Diploma nurses enrolled in RN-to-BSN/MSN programs may be considered)

Experience: Minimum 2 years of relevant nursing experience within the past 5 years

Certifications/Licensure:

  • Current RN license in South Carolina
  • National nursing certification in specialty (required within six months of hire)
  • OCN Certification required for Oncology Care Manager
  • Med/Surg Certification required for 7th South Medical Care Manager

Additional Requirements:

  • Completion of yearly safety training and mandatory classes
  • 20 hours of continuing education annually within specialty
  • Strong conflict resolution and problem-solving skills

Key Responsibilities

  • Develop and oversee holistic patient care plans in collaboration with healthcare teams
  • Provide leadership, guidance, and mentorship to nursing staff
  • Advocate for patients and families, ensuring comprehensive care and education
  • Utilize critical-thinking and clinical judgment for decision-making
  • Coordinate patient flow, admissions, transfers, and discharges
  • Support shared governance initiatives and quality improvement efforts
  • Ensure adherence to infection control, safety, and nursing practice standards
  • Facilitate multidisciplinary collaboration to optimize patient outcomes
