JOB DESCRIPTION AND QUALIFICATIONS (This position will travel within the Cincinnati and surrounding areas)


Position Overview:

We are seeking a compassionate and skilled Therapist to provide high-quality mental health services to residents live live in skilled nursing and assisted living environments. The ideal candidate will be a Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or another equivalent licensed mental health professional. The Assessor will provide biopsychosocial, treatment planning, and case management, to clients in a supportive and professional environment.

In this role, you will work closely with clients to assess their mental health needs, develop personalized treatment plans, and provide evidence-based therapeutic interventions. You will also collaborate with a multidisciplinary team to ensure that clients receive holistic care tailored to their individual needs.

Key Responsibilities:

  • Individual Therapy (small caseload): Provide therapeutic services through individual counseling sessions to clients experiencing mental health challenges.
  • Assessments: Conduct comprehensive assessments (including intake, diagnostic, and risk assessments) to understand client needs and determine appropriate treatment.
  • Treatment Planning: Develop individualized treatment plans in collaboration with clients, addressing their mental health needs and goals. Regularly evaluate and modify treatment plans as necessary.
  • Evidence-Based Interventions: Use evidence-based therapeutic techniques (e.g., Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, Solution-Focused Therapy, etc.) to provide effective treatment to clients.
  • Case Management: Assist clients with coordinating services, connecting them to additional resources or community services as needed, and ensuring follow-up care.
  • Documentation and Record-Keeping: Maintain accurate and timely documentation of client sessions, treatment plans, progress notes, and any other necessary reports in compliance with agency policies and legal requirements.
  • Collaboration: Work closely with other professionals, including physicians, social workers, and case managers, to ensure that clients receive comprehensive care and support.
  • Crisis Intervention: Respond to clients in crisis, providing immediate support and developing safety plans as needed.
  • Client Advocacy: Advocate for clients within the organization and the community, helping to secure resources, support, and services that are in the best interest of the client.
  • Continuing Education: Participate in regular professional development, training, and supervision to stay current with therapeutic practices and maintain licensure.

Qualifications:

  • Education:
    • Bachelor's or Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or a related field.
    • Current and valid Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or equivalent licensure required.
  • Experience:
    • Minimum of 1 year of experience providing therapeutic services, with a preference for experience in counseling/therapy.
    • Experience in individual, therapy is preferred.
    • Familiarity with Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), or other evidence-based therapeutic techniques is a plus.
  • Skills:
    • Strong interpersonal and communication skills.
    • Ability to build rapport with clients and work effectively as part of a multidisciplinary team.
    • Knowledge of mental health diagnoses, treatment modalities, and ethical practices.
    • Exceptional organizational skills and the ability to manage multiple client caseloads.
    • Familiarity with electronic health record systems and the ability to maintain detailed clinical documentation.
  • Other Requirements:
    • Ability to work independently and manage a caseload effectively.
    • Availability for on-call or emergency services, as needed.
    • Flexibility and a willingness to work in a dynamic and evolving environment.
