About Job

Are you a recent graduate with a background in Business or Healthcare Management? Do you have a passion for supporting seniors and improving the way they receive care at home? Ready2Nurse, a 5-star Medicare-rated concierge home health agency, is looking for a Junior Community Liaison to join our mission-driven team in Santa Clara County.

About Us:

Founded in 2018, Ready2Nurse was created with a singular focus: to provide compassionate, customized home health care that empowers seniors to safely and comfortably age in place. We are proudly FNP-owned and operated, bringing advanced clinical insight and hands-on leadership to every aspect of our care.

We operate outside the traditional franchise model to offer truly personalized care that addresses both the clinical and functional needs of our patients. Our team is a healthy mix of professionals and patients who thrive in a supportive, high-quality care environment.

As a Junior Community Liaison, you’ll play a vital role in connecting referral sources, families, and community partners with our services. You’ll help promote our unique concierge model and build relationships that support seniors in remaining safely at home. You’ll work directly with healthcare professionals, senior living communities, and families to spread awareness of our services and values.

Comprehensive training will be provided to ensure you’re fully supported and confident in your role. Whether you're new to the field or looking to grow your experience in healthcare outreach, we’re committed to setting you up for success.

Responsibilities:

  • Serve as a key point of contact between Ready2Nurse and referral sources, patients, and families
  • Educate the community on our personalized home health services
  • Support the coordination of care referrals and follow-up
  • Collaborate with our clinical team to ensure smooth transitions of care
  • Represent Ready2Nurse at community events and professional meetings

Qualifications:

  • Bachelor’s degree in Business, Healthcare Administration, Public Health, or a related field
  • Excellent interpersonal and communication skills
  • Passion for elder care and commitment to community-based service
  • Highly motivated, organized, and a team player
  • Bilingual a plus (but not required)

Why Join Us?

  • Be part of a mission-driven organization with a 5-star Medicare quality rating
  • FNP-owned and operated – led by clinical insight and personalized care
  • Work in a non-franchise, patient-centered agency
  • Comprehensive training provided to ensure your success
  • Opportunity to grow in the fields of community health and healthcare operations
  • Collaborate with a team dedicated to innovation and senior care excellence

Salary: $55,000 - $75,000

If you’re driven to make a difference in the lives of seniors and eager to grow your career in home health, we’d love to hear from you.
