Salary Range: $50,066 to $71,385.60 + benefits

Job Overview
We are seeking a compassionate and detail-oriented PACE Home Care Coordinator to support our interdisciplinary team at the Chula Vista location. In this hybrid field/office role, you will coordinate unskilled home-based care services—including meal delivery, caregiver visits, and durable medical equipment—for seniors enrolled in the PACE program. You’ll advocate for participant needs, conduct home safety evaluations, and ensure services are delivered in alignment with participants' goals and care plans. Most days include 1-3 home visits (30-60 minutes each), with the remaining time in-office.

Requirements to Apply
- Current California CNA and Home Health Aide (HHA) certifications
- CPR and First Aid certification (American Heart Association)
- Valid California Driver’s License
- At least 1 year of experience with frail or elderly populations
