Orientation (classroom): $165.00 per day.

Our hospice and palliative care * We provide services to help patients and families manage life-limiting conditions, whether in the community or facility-based settings.
- Provide psychosocial support and social work services to hospice patients and families in inpatient, home care, or nursing home settings, including pre- and post-bereavement.
- Serve as social work member of the hospice IDG and liaise between hospice staff and inpatient/nursing home staff.
- Develop social work plans of care, goals, and recommendations based on patient/family needs for direct admissions to inpatient units or nursing homes.
- Create social work and discharge plans when appropriate for inpatient admissions.
Minimum ****
- Masters Degree in Social Work from an accredited school.
- At least one year of healthcare social work experience.
- Knowledge of Word and Excel preferred.
- NY State MSW licensure (LMSW or LCSW).
- Valid NYS Drivers license; car may be required based on job location.
- Ability to work within an interdisciplinary team.
- Proficient clinical data entry skills.
