HealthCare Support is actively seeking a Care Transitions Nurse Manager – RN to fill an opening with a prestigious Healthcare company in The Bronx, NY
Daily Responsibilities for Care Transitions Nurse Manager – RN :
- Will make field visits but will not carry their own caseload.
- Assisting and overseeing some of the more complex cases – acting as a SME for Community Health Nurses in the field
- Communicate with interdisciplinary teams, social workers, and dietitians, helping coordinate that care
- Monday – Friday 8:30 am – 5:30 pm I On-Call Rotation
- This is a field position*
- Active NY Registered Nursing license
- 3+ years' experience as a Registered Nurse / RN
- 2+ years of recent experience at a Certified home health agency (CHHA)
- 1+ year of recent supervisory experience
- Medical, Dental, Vision
- HAS/FSA
