Join our Care Management/Social Work team as a Patient Care Coordinator, where you will play a vital role in planning, coordinating, and evaluating patient care across various service lines.

In this position, you will work collaboratively with interdisciplinary teams to ensure the highest quality of care while optimizing clinical and financial outcomes.

Your responsibilities will include:

• Facilitating coordinated patient care with physicians and team members both pre and post-operatively.
• Monitoring clinical processes to ensure timely, effective interventions that lead to optimal patient outcomes within appropriate lengths of stay and budget constraints.
• Playing an active role in discharge planning by assessing needs, coordinating resources, and evaluating discharge effectiveness, starting from admission.
• Utilizing your specialized knowledge to guide the development of individualized multidisciplinary care plans that span pre-hospitalization to post-discharge transitions.
• Leading care conferences for patients with complex health issues to ensure a comprehensive approach to treatment.
• Providing education to patients and families to support continuity of care during discharge.
• Demonstrating expertise in referring to community resources for patient support and services.

• Partnering with healthcare teams to implement strategies that effectively reduce resource consumption while enhancing patient health status.
• Identifying the educational needs of healthcare professionals and providing relevant training opportunities.

• Ensuring that all communications regarding documentation are accurately transmitted to community agencies.
