RN Care Manager (Hybrid) – $2,800/Week | Seattle, WA
Location: Seattle, WA 98144
Weekly Gross Pay: $2,800/week
Start Date: September 8, 2025
Assignment Duration: 13 Weeks
Schedule: Monday–Friday | 5x8s | 8:00 AM – 4:00 PM
Work Setting: Hybrid (2 Days Onsite / 3 Days Remote)
Charting System: Epic (Experience REQUIRED)
Patient Population: Adolescents, Adults, Geriatrics
Daily Census: 15 patients | ~8 visits per day
Position Overview:
We are seeking an experienced Registered Nurse Care Manager to support transitional care coordination in a hybrid outpatient setting. This role is vital in reducing ED visits and readmissions by managing discharges and coordinating care for patients with chronic conditions. It offers a unique opportunity to work both onsite (2 days) and remotely (3 days) each week in Seattle, WA.
Key Responsibilities:
- Manage post-discharge and ED visit care coordination
- Complete medication reconciliation and follow-up appointments
- Apply InterQual/Milliman guidelines and regulatory protocols (CMS, HIPAA, NCQA, etc.)
- Utilize Epic for documentation and tracking
- Collaborate with interdisciplinary teams including Social Services
- Focus on chronic disease management and transitions of care
Requirements:
- License/Certifications: Active RN license, BLS (Required)
- Experience:
- Minimum 1 year in Acute Care, LTAC, Rehab, or Skilled Nursing settings
- Strong familiarity with case management/utilization review
- Epic proficiency required
- Must have reliable personal transportation (public transit not accepted)
- Must be able to pass background and credentialing prior to start
- Open to local and non-local candidates
Additional Details:
- No weekends, no call
- Self-scheduling model
- No float requirement (unless within skillset)
- Orientation: Fully supported by the clinical and administrative team
- Patient Diagnoses: All IP discharges (excluding L&D and oncology), all ED visits focused on chronic care
