Care Coordinator II

YOU Inc. is seeking a compassionate and organized Care Coordinator II to support individuals and families accessing behavioral health and social services. This role is essential in ensuring continuity of care, facilitating warm hand-offs, and coordinating services across outpatient clinics and community-based resources. The Care Coordinator II will work closely with clinicians and community partners to promote wellness, stability, and access to long-term supports.
Pay:Â
$24.04 - $25.00 Hourly
Sign-on Bonus:Â
$5,000 (paid quarterly over 18 months)

Responsibilities
- Conduct initial screenings and needs assessments in collaboration with behavioral health clinicians.
- Coordinate care transitions between outpatient, urgent care, crisis, and inpatient services.
- Facilitate discharge planning and arrange follow-up services within required timeframes.
- Initiate warm hand-offs to behavioral health providers, primary care, and community supports.
- Support the development of individualized care coordination plans.
- Maintain communication with key partners including CBHCs, ACOs, Family Resource Centers, and schools.
- Ensure timely and accurate documentation in compliance with HIPAA and 42 CFR Part 2.
- Participate in team meetings, supervision, and quality improvement initiatives.
- Monitor follow-through on referrals and help remove barriers to care (e.g., transportation, insurance).
- Provide culturally and linguistically appropriate services.
- Assist with data collection and reporting related to service access and delivery.
Education & Experience:
- Bachelor's degree required (social work, psychology, human services, or related field).
- 1–3 years of related experience.
- Valid Class C or D driver's license and reliable transportation required.
- Â CANS certification required.
- Strong communication, empathy, and interpersonal skills.
- Cultural competence and ability to work with diverse populations.
- Excellent documentation and organizational skills.
- Ability to work independently and collaboratively.
- Bilingual/bicultural candidates.
- Experience with behavioral health or social service coordination.
- Familiarity with MassHealth and Massachusetts behavioral health systems.
