Tricare Home Health Services Inc. is a locally owned agency that’s been serving San Antonio with heart since 1996. We’re looking for a Licensed Vocational Nurse (LVN) or Registered Nurse (RN) to join our in-office care coordination team.

This is a full-time, Monday–Friday role (no field visits) focused on helping our agency run smoothly and stay compliant — while providing support to our field staff and admin team.

???? What You’ll Be Doing:

???? Care Coordination & QA Review charts, update care plans, ensure timely signatures, and help us stay survey-ready

☎️ Follow-Up & Communication Track orders, follow up with physicians, communicate with patients and families

????️ Compliance Support Double-check documents, verify signatures, and prepare for audits

???? Intake Coordination Review new referrals, initiate Plan of Care, confirm insurance, and coordinate with clinicians and schedulers

???? Team Support Assist the Director of Nursing and admin team with clinical questions, patient records, and daily operations

???? What We’re Looking For:

Active LVN or RN license in the state of Texas

1–3+ years of nursing experience (home health or intake experience preferred)

Highly organized, great with follow-up, and detail-driven

Strong communication and documentation skills

Comfortable with EMR systems (we use Axxess)

Spanish-speaking is a plus, but not required
