The Behavioral Health Home Hospital Research Social Worker/Case Manager will support an innovative care delivery model.
The program offers a subset of patients an alternative to traditional inpatient psychiatric hospital admission.
Patients can receive highly specialized psychiatric care services in their homes.
The Research Social Worker/Case Manager will provide quality clinical services to patients in various community settings.
Patient care is delivered within organizational philosophy, policy, and standards of nursing practice.
Responsibilities are performed in accordance with established policies and standards of practice.
Timely electronic documentation and interdisciplinary communications will be utilized.
The Research Social Worker/Case Manager will collaborate with other professionals to provide clinical services.
These services address psychosocial, environmental, age-specific, and cultural issues.
The goal is to maximize emotional, social, and physical well-being and effective use of community resources.
Clinical social work techniques and theory will be employed to provide consultation, assessment, and treatment interventions.
Advocacy for patients and families will be a primary focus.
The Research Social Worker/Case Manager can expect to see patients in person in their homes or remotely.
Up to four patients will be managed at a time.
Hospital-level treatment will be provided to patients in the community.
This role is ideal for someone interested in a temporary position working on a research trial.
The trial aims to broaden treatment opportunities for patients with active acute psychiatric conditions.
Position will begin on a part-time basis with required training.
There will be approximately 12 hours of training in the initial month.
In subsequent months, the schedule will adjust to approximately 14 hours per week depending on caseload.
The team is launching a pilot randomized controlled trial to test acceptability and feasibility of providing acute psychiatric care at home.
Currently, home hospital programs exclude patients with active psychiatric conditions.
This trial will evaluate the feasibility and acceptability of the Behavioral Health Home Hospital model.
The model provides in-person nursing care twice a day, along with remote support from psychiatric professionals.
Additional clinical care team members will offer care either in person or remotely.
Patients admitted as part of the trial will need a caregiver present.
Support can also be provided by home health aides and mental health specialists.
Medication will be dispensed by the clinical care team.
Continuous monitoring will be conducted, and patients can join in-patient therapy groups remotely.
The Research Social Worker/Case Manager will be a key member of the clinical care team.
They will provide consultation, assessment, treatment interventions, and advocacy for patients and caregivers.
