Role Description

This is a part-time role for a Care Manager/Social Worker. The Care Manager/Social Worker will be responsible for providing case management services, training, and support for elder care. Day-to-day tasks include conducting assessments, developing care plans, coordinating with healthcare providers, and offering advocacy and support to seniors and their families. This is a hybrid role based in Plymouth, MA, with some work from home acceptable.


Qualifications


  • LCSW or equivalent licensure preferred
  • Care Management and Training skills
  • Elder Care and Social Services experience
  • Knowledge in Medicine related to elder care
  • Excellent communication and interpersonal skills
  • Ability to work independently as well as in a team
  • Bachelor's or Master's degree in Social Work or related field
  • Experience in care management or social services is a plus
