PACE Social Worker (MSW)
Location: San Ysidro, CA 92173
Schedule: Monday – Friday, 8:00 AM – 5:00 PM (Fully Onsite)
Assignment Duration: Approximately 5 months
Pay Rate: $47.25/hour

Position Overview:

San Ysidro Health is seeking a compassionate and skilled Master’s-level Social Worker (MSW) to join our PACE (Program of All-Inclusive Care for the Elderly) team. This is a temporary full-time onsite role where you'll support older adults in accessing the care and resources they need to thrive while maintaining their independence.

Responsibilities Include:

• Conduct comprehensive psychosocial assessments
• Develop and coordinate individualized care plans
• Provide ongoing case management and supportive counseling
• Collaborate with interdisciplinary care teams to support patient needs
• Advocate for patient rights and connect participants with community resources

Required Qualifications:

• Education: Master’s Degree in Social Work from an accredited institution
• Experience: Prior experience working with older adults or in a healthcare/social work setting preferred
• Certification:
• Valid California Driver’s License
• Access to an insured personal vehicle (travel required between sites)

Language Requirement:

• Bilingual (English/Spanish) strongly preferred
• Proficiency in Medical Spanish highly valued
