Housing Case Manager

BHcare is seeking a compassionate and proactive Housing Case Manager to support individuals with a history of opioid use who are experiencing or at risk of homelessness. This is a vital role helping clients navigate the housing system, connect with resources, and build stability for the future.

BHcare is a Certified Community Behavioral Health Clinic offering comprehensive, person-centered care to individuals and families navigating mental health, substance use, and housing challenges. Our multidisciplinary teams work across communities to empower clients through evidence-based practices and unwavering support.

Position Summary

The Housing Case Manager engages clients with opioid use histories and supports them in obtaining and maintaining stable housing. This position blends outreach, advocacy, and case management to remove barriers and foster long-term stability. Working collaboratively with internal teams and community partners, the Housing Case Manager plays a key role in promoting recovery and reducing homelessness.

Key Responsibilities

Client Engagement & Support
  • Build trust and maintain regular, supportive contact with clients.
  • Assist individuals in identifying housing options and navigating the rental process.
  • Support clients in creating personalized housing stability and recovery plans.
  • Provide connections to community-based services and assist with transitions.
  • Respond flexibly to clients’ evolving needs and crises.

Collaboration & Coordination
  • Work in partnership with internal programs, housing providers, and local agencies.
  • Facilitate access to housing, benefits, employment, and healthcare resources.
  • Participate in regular case reviews, program meetings, and cross-functional collaboration.

Documentation & Reporting
  • Maintain timely, accurate documentation in the electronic health records system (EHR).
  • Monitor progress toward client goals and report program outcomes.
  • Comply with all documentation and data-entry standards.

Program & Community Involvement
  • Represent BHcare in community meetings, housing-related events, and with partner agencies.
  • Assist with transportation for clients, when needed.
  • Follow harm reduction and Housing First principles in all service delivery.

  • High School Diploma or equivalent required, bachelor’s degree preferred.
  • Bi-lingual Spanish preferred.
  • Experience working with persons with Mental Health and/or substance use disorders.
  • Experience working with the homeless or marginalized population.
  • Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
  • Training in harm reduction, motivation interviewing, de-escalation techniques
  • Demonstrated ability to work with diverse communities.
  • Understanding of Housing First philosophy.
  • Working knowledge of employment, housing and human services in Greater New Haven.
  • Must have and maintain a valid driver’s license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
