Social Worker 2
Location: Lexington, TN (with coverage of multiple sites)
Duration: September 1, 2025 – June 30, 2026
Schedule: Monday – Friday, 8:00 AM – 4:30 PM
Pay Rate: $30/hr (1099) OR $28/Hr on W2
Position Overview
Seeking a dedicated Social Worker 2 to provide support at the Henderson County Health Department and affiliated sites. This position is integral to the Clinical Care Team, assisting patients in overcoming barriers related to social determinants of health (SDOH) and ensuring seamless access to community resources, specialty care, and prescription assistance programs.
Qualifications
- Bachelor’s degree in Social Work or related field (Master’s preferred).
- Valid Tennessee Social Work license (or ability to obtain).
- Experience in healthcare, mental health, or community services preferred.
- Strong knowledge of community resources and social service programs.
- Excellent communication, organizational, and problem-solving skills.
- Valid driver’s license and personal vehicle for travel between sites.
Key Responsibilities
Social Support & Resource Navigation
- Address SDOH concerns such as housing, food, and transportation needs.
- Conduct social needs assessments and maintain an up-to-date resource directory.
- Refer patients to appropriate community programs and services.
- Collaborate with fellow social workers and primary care providers to coordinate support.
Case Management
- Facilitate hospital discharge planning and follow-up care.
- Retrieve and organize medical records from hospitals and specialists.
- Assist patients in obtaining medical equipment through community partnerships.
- Re-engage patients who are lost to follow-up and track care plans.
Specialty Referral Coordination
- Manage referrals for specialty services and imaging not covered by BCS.
- Track and obtain external provider records for continuity of care.
Prescription Assistance
- Support patients in accessing low-cost medication programs (CoverRx, RxOutreach, etc.).
- Liaise with pharmaceutical companies and community resources for medication support.
Additional Responsibilities
- Participate in regional initiatives involving social work and patient support.
- Maintain thorough documentation in patient records.
- Attend regional and community meetings as needed.
- Build strong partnerships with local agencies to reduce care barriers.
Additional Information
- COVID-19 vaccination (including booster) required.
- Candidates must not have worked (perm or travel) with any Beth Israel facility in the past 6 months.
- Local candidates preferred.
Job Type: Contract
Pay: $28.00 - $30.00 per hour
Expected hours: 40 per week
