Weekend Rotation Required | Epic Charting | Acute Hospital Setting

???? Facility: Providence Health & Services – Torrance, CA

Shift: Day Shift (5x8s), 8:00 AM – 4:30 PM

Weekend Requirement: Every other weekend & holiday coverage

✅ Position Requirements:

• MSW – Master’s in Social Work (Required)
• Minimum 2 years of recent acute care hospital case management experience
• Travel experience required
• EPIC charting system proficiency
• Must be able to perform care coordination and discharge planning
• Strong knowledge of:
• Community resources (Medicaid, food stamps, medical transport)
• Substance abuse, homelessness, mental health, POLST
• Advance directives, protective services, hospice
• 51/50 holds, family therapy, biopsychosocial assessments

???? Key Responsibilities:

• Manage caseload up to 25 patients across multiple units on weekends
• Conduct biopsychosocial assessments & intake screenings
• Participate in treatment planning & care conferences
• Ensure continuity of care and safe discharges
• Coordinate with interdisciplinary team including IV, PT, RT, pharmacy, and more
