Schedule: Monday – Friday | 8:00 AM – 5:00 PM | Fully Onsite
Pay Rate: $40 – $45 per hour (based on experience and qualifications)
Start Date: TBD

Job Overview:

We are seeking a compassionate and qualified Masters Social Worker (MSW) to support an outpatient PACE clinic. This is a full-time, onsite role focused on providing psychosocial support, care planning, and coordination services to older adults within a community-based care model.

Key Responsibilities:

Conduct initial and ongoing psychosocial assessments
Develop and implement care plans in collaboration with an interdisciplinary team
Provide counseling and crisis intervention to patients and families
Coordinate community resources and support services
Advocate for participants’ rights and care needs
Maintain accurate and timely documentation

Qualifications:

Education: Master’s Degree in Social Work (MSW) from an accredited institution
Experience: Prior experience in outpatient, geriatric, or PACE programs preferred
License: Valid CA driver’s license and access to an insured vehicle required
Language: Bilingual English/Spanish preferred; medical Spanish strongly desirable
Certifications: Current BLS preferred, if required by site policies

Skilled in assessments, counseling, care planning, and interdisciplinary teamwork
Ability to manage a caseload and adapt to evolving patient needs
