Description
TheCareManagerposition will focus on provision of person-centered services and comprehensivecare
managementincluding planning and coordination of all physical, behavioral and support services to increase
independent living skills and positive community involvement for enrolled participants. TheCareManager does this through guiding person-centered goals and facilitating meaningful connections to natural and formal support networks.
TheCareManager will participate in ongoing professional development to enhance knowledge of community resources, process to support access of services and benefits, and to refine treatment documentation and approach.
TheCareManager position operates primarily in the community, while theCareManager should also be skilled at working at the agency office and virtually via telehealth. TheCareManager position utilizes electronic health record to document work on a daily basis and should be experienced and efficient working within electronic systems.
Qualifications
EDUCATION & EXPERIENCE:
•Master's Degree in a human services related field OR
•Bachelor's degree in a human services related field and 1 year relevant experience working with the
target population preferred OR
•High School Diploma or GED with a minimum of 3 years of experience working with the target
population OR
•Certified Peer Specialist (CPS) or Certified Family Specialist (CFS)
Demonstrated success in the following areas:
•Ability to manage time to meet all deadlines.
•Ability to recognize and identify symptoms of mental illness and suicide and risk potential and
respond appropriately.
•Good verbal and written skills.
•Bilingual (English/Spanish) is preferred but not required.
Physical Demands:May have to lift and carry up to 20 lbs. 80-90% of the time the employee is standing,
walking, kneeling, or reaching in an administrative capacity. Requires full range of body motion. Must have
fine motor skills to perform typing and writing. Emotional stability to handle stressful situations including
unexpected deadlines. Vision and hearing sufficient to perform essential functions. Ability to speak and write.
A valid driver's license is needed for community work.
