This role is perfect for an RN or Social Worker who’s passionate about advocacy, education, and making healthcare more accessible and coordinated.


Qualifications

  • Education: RN or LMSW required
  • License: Active Texas RN, LMSW, or LCSW license
  • 2+ years in care management, care coordination, or related field
  • Experience with interdisciplinary teams preferred
  • Strong communication, customer service, and computer skills


Key Responsibilities

  • Identify high-risk patients and coordinate individualized care plans
  • Conduct proactive outreach via phone, online, or in-person
  • Collaborate with healthcare teams to ensure effective transitions of care
  • Educate patients on chronic disease management and medication adherence
  • Connect patients to community resources and support services
  • Monitor and promote adherence to care plans
  • Reduce ER visits and hospital readmissions
  • Support health literacy, self-management, and shared decision-making
  • Refer patients to additional care team members as needed
  • Uphold safety standards and promote professional growth
