Position Summary:
We are looking for a caring, reliable, and community-driven Patient Care Coordinator who is passionate about helping local families live stronger, longer, and healthier lives. You will serve as the monthly point of contact for patients receiving treatment, ensuring that they receive their medications, lab work, and appointments on time — and most importantly, feel supported every step of the way.

This position is perfect for someone who wants to make a real difference in their hometown, building relationships with patients, and helping to restore strength and balance in families through ongoing care and communication.

Core Responsibilities:

• Manage monthly patient care cycles, including medication orders and refills, lab orders and results, follow-up appointments, and billing support.
• Reach out to every patient personally each month to check in, answer questions, and provide guidance.
• Serve as a reliable, local advocate to make sure every man, woman, and family member in treatment knows they are supported.
• Track patient progress, lab results, and important action items using our secure online system.
• Work closely with our providers and clinical staff to keep each patient's care moving forward without delay.

Compensation Structure:

• $20 per patient per month under your care
• Your income will grow as more families in town receive care — the more you serve, the more you earn
• Additional rewards may be offered for excellence, retention, and patient satisfaction

Time Commitment & Flexibility:

• Part-time to start — just a few hours a day as we grow
• Flexible schedule that works around your family or other responsibilities
• This position is designed to grow into a full-time, stable role as our community presence expands
