OVERVIEW:
The social worker will work with residents in the nursing home by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs.
RESPONSIBILITIES:

The social worker will work with residents in the nursing home by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs.
RESPONSIBILITIES:
- Working collaboratively with interdisciplinary team members
- Completing BIMS and Mood Assessments
- Documenting in medical chart
- Knowledge of Sigma Care Electronic Medical Record system
- Ability to work well with Residents and families
- Timely MDS and care plan completion
- Coordinating and implementing safe discharges
- Ability to problem solve
- LSW, LCSW, or LICSW in the state of New York
- 1-year experience in SNF (preferred)
- Prior work with elders in a community or long-term care setting is preferred
- Must have strong skills in communication, assessment and social work methods and techniques.
- Must have a working understanding of social systems along with the ability to implement pertinent state and federal regulations.
