The Care Coordinator at the Sinai Post Discharge Clinic serves in a dual-capacity role, integrating clinical expertise as a Licensed Clinical Social Worker (LCSW) with oversight of day-to-day administrative functions. This role is essential to ensuring smooth transitions of care following hospital discharge by coordinating timely follow-up, connecting patients to community resources, and supporting the operational workflow of the clinic. The position also supports regulatory compliance and performance metrics, helping to ensure patients receive appropriate post-discharge care while being informed of their rights, responsibilities, and available services.

Requirements
- Master degree - Social Work
- Current Maryland LCSW, LCPC or Psychology license
- American Heart Associate – Basic Life Support Certification (BLS) (Preferred)
- Care Manager Certification (Preferred)
