- Conduct home visits within your assigned area to assess safety, monitor health changes, and offer clinical coaching, including medication reviews.
- Follow an established care plan and provide ongoing coaching through our digital app, offering resource referrals and best practice advice.
- Build trusting relationships with caregivers to encourage positive changes and help reduce burnout.
- Maintain accurate and timely documentation, ensuring compliance with standards and regulations.
- Take on additional projects and duties as needed.
- An active Registered Nurse (RN) License in the state of employment.
- At least two years of recent experience in direct care for elders or adults with disabilities.
- Capability to travel within your assigned area (a valid driver's license and auto insurance are a must).
- Experience in case management, care planning, and assessment, particularly in eldercare, disabilities, or complex medical needs.
- Proficiency with common software and web-based applications, including MS Office and case management systems.
- Bilingual skills are a plus and can influence compensation.
